You must add-a-line of service to an existing Verizon account either with a new smartphone or activating 2nd number on a compatible e-SIM or dual-Sim device (“Eligible Device”) between 12/26/2023 and 12/31/2023 to qualify for this promotion via mail-in rebate request (the “Offer”). Your mail-in rebate request will be considered properly submitted once you have completed the following: (1) completed, dated, and mailed this Bill Credit Form to Victra’s address listed above no later than 2/14/2024; (2) attached your purchase receipt (original or copy), which must include at least one Eligible Device and must be dated between 12/26/2023 to 12/31/2023; and (3) activated Verizon Wireless service for an Eligible Device listed on your attached purchase receipt which must remain activated at the time Victra receives your rebate request in order to be eligible for the bill credit (together, the “Rebate Submission Criteria”). You will receive the $65.00 credit as a one-time bill credit applied to your Verizon Wireless account within 60 days from the date you have fully satisfied the Rebate Submission Criteria.
